This is an exciting opportunity to join our 0-25 Disability Team which is a multi-disciplinary team that includes Social Workers, Social Work Assistants and Specialist Health Colleagues. We work with disabled children, young people and their families across Carmarthenshire and our through age model allows us to provide a seamless service to some of the most vulnerable in our community.
The team is an established, dedicated and skilled team who are passionate about supporting children, young people and their families. You will work alongside Social Workers, health and education professionals to provide a comprehensive service to disabled children, young people and their families.
The role will require you to undertake assessment, care planning and direct interventions with families.
This is a challenging yet rewarding role and we welcome applications from individuals who have experience working with disability or supporting children, young people and families.
We are one of the few local authorities that have consistently reduced the number of looked after children within their county and we have done this through innovation in our social work teams and investment in preventative services as well as working to empower and support families to become more resilient.
