1 x Permanent - 37 hours per week.

1 x Maternity Cover – 37 hours per week (Fixed Term – 12 Months)

Band 10 - 12 £28,226 - £38,553 pa

The Early Intervention and Hospital Discharge Team have an exciting opportunity for a Social Worker to join our dynamic and dedicated team in the early intervention services.  The team in based within University Hospital of Hartlepool with the Integrated Single Point of Access (iSPA). 

The team is the initial point of contact for all adults and aims to promote independence throughout intervention, whether this been a support service, advice/guidance or signposting.  Working with a strengths based approach alongside colleagues within health and social care, the team work closely with the Community Hubs within the town to maintain awareness of support networks available with a Community Led Support approach. 

You should have recent relevant experience of working in a Social Care Management setting dealing with enquiries from the public or agencies. A sound knowledge base and understanding of roles and responsibilities within Social Care is essential, ideally including knowledge of current developments and service pathways.


Under the umbrella of Safeguarding you will undertake social care assessments and support planning; monitor and review packages of support. You will need good administrative skills and a proven experience of developing, maintaining and inputting information on electronic based systems. You will be able to demonstrate sound analytical skills and affective decision-making within appropriate timescales; demonstrate a person-centered approach and be able to act in a tactful and sensitive manner with people who access services.

The successful applicant will receive regular supervision and an Annual Review, be well supported and have opportunities in respect of their career development, including experiencing a variety of roles within adult social care.
