Working under the principles and values of Community Led Support, offer an initial contact service for Adult Services that focuses on having good, strength based, solution focused conversations with people that centre on improving their outcomes, reducing isolation, empowering them to make effective choices and exploring opportunities and activities.
Key Relationships:
Based in the Support Hub, the post-holder will support access to appropriate services and help to reduce inequalities within the Hartlepool populations with:
Local Authority managers and staff
People who use adult servicesCarers
Voluntary and Community Sector (VCS) groups
