We are looking
for highly motivated qualified Social Workers with proven knowledge and
experience in Older People's social work. You will be a key member of the Older
People team, carrying out quality social work assessments and acting as
decision makers in accordance with the Main Principles of the Mental Capacity
Act (2005), to enable vulnerable older people over the age of 65 in Wrexham to
remain in their home or in the most appropriate environment taking into account
their safety and wellbeing.
In the role you
will work in partnerships with key stakeholders including the NHS, local
authorities or independent sector providers, families and unpaid carers to
ensure quality care and support is provided to older people in need. You will
have the opportunity to support older people with a range of needs including
those relating to physical and mental health and will have the skills to hold
conversations about "what's important" to inform results-oriented
assessments and support provision.
The Older
People team is involved in a number of projects to improve services for our
citizens including integrated working across the whole system. These projects
and partnerships across Wrexham benefit Older People, and so it's a great time
to join the team and contribute to shaping the services of the future. The Team
is proud of its strong multidisciplinary relationships, and adopting a Local
model enables practitioners to build these relationships across statutory and
third sector agencies and become experts on the services available in their
geographic area.
