You will have the opportunity to support older people with a range of needs including those relating to physical and mental health and will have the skills to hold conversations about "what's important" to inform results-oriented assessments and support provision.

The Older People team is involved in a number of projects to improve services for our citizens including integrated working across the whole system. These projects and partnerships across Wrexham benefit Older People, and so it's a great time to join the team and contribute to shaping the services of the future.
