An exciting opportunity has arisen to extend the delivery of enablement and progression services across Social Care. The post-holder will be located within the Project Management Team to lead on empowering individuals to achieve greater independence and control over their lives.
The post holder will be required to assess individuals and/or services to identify development needs and opportunities reducing reliance on paid support. The post holder will liaise with a variety of professionals, services and organisations to enhance and achieve improved enablement and progression outcomes. This will include working with communities and networks to enhance and develop opportunities. The post holder will monitor and record outcomes and success achieved to ensure independence is progressed / maintained.
Based in the Community you should have:
- Experience in developing & co-ordinating enablement and progression
support / services.
- Experience of working with vulnerable adults / people with disabilities / complex needs.
- The ability to work creatively on your own initiative or as part of a team.
- Good
communication, interpersonal skills and record keeping.
- A systematic approach to work including the ability to prioritise work initiatives within clearly defined outcomes.
