This is an exciting opportunity for a Social Worker to join Adults Single Point of Access.
The Adults Single Point of Access receive all new contacts and referrals for those citizens 18+ within Wrexham County Borough Council. The Social Services and Well Being (Wales) Act 2014 provides the legislative framework regarding the provision of information, advice and assistance and identifying needs for care and support of Adults. Adults Single Point of Access are pivotal in ensuring Adults are supported to achieve their wellbeing outcomes through outcome focussed conversations, providing information, advice and assistance; and/or identifying whether further assessments of care and support needs are required.
There will be a focus on development for this team, over the coming months, to ensure that there are strong networks made with other statutory and community based teams within the third sector; as multi agency working is essential for assessments completed by Adults Single Point of Access.
The successful applicant will be working alongside a team of First ContactAdvisors, Social Care Assessors, Social Workers, and Occupational Therapist.
The ideal candidate will be a Qualified and Registered Social Worker with Social Care Wales. They will evidence good knowledge and application of legal duties to inform assessment and intervention, have excellent communication skills, able to work as part of a team and have previous experience of working with vulnerable adults.

The team are based at Crown Buildings. This role also offers the opportunity for hybrid working.
An Enhanced DBS check and Social Care Wales registration is required for this position.
