is a really exciting time for Hartlepool substance misuse services. We have
recently created an integrated substance misuse service, which includes our
clinical service provided by an external partner and our ‘in-house’ care
co-ordination service.
If you are successful you will support the leadership team in both developing and delivering a service we hope can improve the lives of people in Hartlepool.
As a key member of the team, you will be responsible for the assessment, planning, implementation and evaluation of planned care for a defined caseload, providing support in your role as a care coordinator, to access services in order to meet individual’s needs/develop strengths, which both reduce harm and support meaningful and sustained recovery.
As care coordinator, you will have responsibility for caseload management of service users who may or may not have a need for medically assisted recovery and for delivering evidenced based individual and group interventions including psycho-social interventions, which not only supports and motivates service users along a recovery pathway, but also delivers positive outcomes for individuals, families and communities.

Applicants should have experience of working with vulnerable individuals with multiple needs and possess a Diploma/BTEC/NVQ3 or equivalent qualification in substance misuse, psychology, counselling, social work or health and social care.
