We have an exciting opportunity for an enthusiastic individual to join our Community Hubs and Wellbeing Section to build on the success of the Hartlepool Support Hub and Community Led Support (CLS) to further develop an excellent service that supports vulnerable and isolated people in the town to have a good life.
Working with the Wellbeing Manager, the successful candidate will work with internal and external partners to continue to develop the Support Hub to provide a range of services that will improve outcomes for people, reduce isolation and empower people to make effective choices. Strength based, solution focused good conversations are the key principles that guide our Support Hub team.
Having excellent interpersonal skills, the job holder will be able to establish good working relationships with all relevant participants and work collaboratively with organisations and,
must be able to work independently and as part of a team, be creative and solution focused, and be confident in delivering support to people.
