Hartlepool is a vibrant and energetic town with growing prospects. As a LocalAuthority we are striving to support the people of Hartlepool to the best they possibly can be and would like to recruit dedicated, driven and committed people to help us achieve that.

We are seeking to recruit a SEND Link Social Worker to work across Early Help and Social Care services in Hartlepool. As a SEND Link Social Worker, you will be supported with regular supervision, comprehensive training and friendly, welcoming colleagues. You will be based in one of our Family Hubs and will be joining us on a journey of change as the Family Hub model builds from strength to strength. Do you want to join us?

If so, you will be a qualified and registered Social Worker who is familiar with the SEND processes and Education, Health and Care Plans that support children and young people with additional needs. You will signpost families at the earliest opportunity and also support your colleagues in both Early Help and Social Care with the SEND processes. You will be an advocate and representative on panels, offer advice and guidance to professionals, and also work with the management team to devise and implement pathways within our Family Hubs.

This post is newly developed and is therefore a great opportunity for you, with the qualifications and qualities you will have, to build on our view that children and young people in Hartlepool will receive the right support, at the right time and is supported by the reforms for SEND and alternative provision changes system in England.
