Using a range of evidenced based tools and techniques the post holder will be responsible for a designated caseload of children and young people of varying needs and complexities.  Through assessment, planning and implementation you will identify needs, develop strengths, build resilience and reduce substance related harm.

 

Other key duties and responsibilities include;

 

·       Provide a range of one-to-one evidenced based interventions including psychosocial interventions to support recovery

 

·       Raise awareness, knowledge and understanding about substances, their use and effects

 

·       Carry out risk assessments and be responsible for the development and implementation and effective review of the plan

 

·       Promote choice, well-being and the protection of all individuals

 

·       Work as part of a team to prevent disengagement of individuals

 

·       To work closely and in partnership with the multi-disciplinary team from across the partnership and external partners and contribute to meetings and plans as appropriate

 

·       Support and signpost individuals to access primary and secondary services across the community to improve and maintain their health and wellbeing needs

  

If you are successful, you will support the leadership team in both developing and delivering a service, we hope can improve the lives of young people in Hartlepool.

 

Applicants should have experience of working with young people with multiple needs and possess a Diploma/BTEC/NVQ3 or equivalent qualification in substance misuse, psychology, counselling, social work, health and social care or youth work
