An exciting opportunity has arisen for an Integrated Community Services Team Lead to join our dynamic and dedicated team in the Hospital Discharge, Safeguarding and Early Intervention Services. The role includes the Integrated Single Point of Access and Early Intervention Services who are based at various buildings including (but not limited to) University Hospital of Hartlepool, University Hospital of North Tees and Billingham Health Centre.
You will need to have recent relevant experience of working in a Social Care Management setting dealing with enquiries from the public and/or agencies. A sound knowledge base and understanding of roles and responsibilities within Social Care is essential, including knowledge of current developments and service pathways.
You will have experience of working in collaboration with colleagues from other statutory, private and voluntary sectors and will be expected to have In-depth knowledge of the Care Act 2014 and the Mental Capacity Act 2005 to ensure compliance of the work within a legal framework.
Under the umbrella of Safeguarding you will supervise the Assistant Team Managers and guide and advise staff undertaking social care assessments, planning, monitoring and reviewing packages of support. You will have good interpersonal and communications skills, with a good base of organisational and administrative skills, proven experience of developing and maintaining services and systems. You will oversee the Discharge to Assess and Intermediate Care budgets, whilst working within appropriate timeframes. You will support staff, working with a range of professionals to ensure positive outcomes for the people of Hartlepool. You will be able to demonstrate sound analytical skills resulting in effective decision-making, demonstrate a person-centered approach and be able to act in a tactful and sensitive manner within a dynamic and fast paced setting.
The successful applicant will receive regular supervision and an Annual Review, will be well supported and have opportunities in respect of their career development, including experiencing a variety of roles within adult social care.
