Social Worker
G09 £30,451 - £32,910 per annum
We are seeking highly motivated qualified Social Workers
with proven knowledge and experiences in Older People’s social work. You will
be a key team member of the Older People’s team, delivering quality social work
assessments and acting as the decision maker in line with the Key Principles of
the Mental Capacity Act (2005), to enable vulnerable older people over 65 years
of age within the Wrexham County to remain at home or in the most appropriate
environment.
The role will see you working in partnerships with key
stakeholders including NHS, local authority or independent sector providers,
families and unpaid carers to ensure quality care and support is provided to
older people. You will have the opportunity to support older people with a
range of needs and be skilled in holding “what matters” conversations to inform
assessments and the provision of outcome focused support.
The Older People’s team is involved in a number of projects
to improve services to our citizens including integrated working across the
whole system. These projects and partnerships across Wrexham are benefiting
Older People, so it’s a great time to join the team and contribute to the
shaping of future services.
An enhanced DBS & Social Council for Wales registration
is required for this position.
