G07 £24,920 - £26,446 

37 hrs per week (Job Share/Part-time hours will be considered)

This is an opportunity to join a busy and friendly team within the Social Services department. The team covers a range of support services – domiciliary care, residential care, supported living, day opportunities and other services from third sector providers. You will support the team to deliver projects that benefit people living with dementia and their unpaid carers. 

Your duties will include provision of practical support to individuals on a one to one or group basis.  Assisting people to make good use of technology (training provided on the technology), support co-production with citizens with dementia and their unpaid carers, engagement and support to providers who’s services support people living with dementia and their unpaid carers.  Support the delivery of the Dementia Action Plan, National Dementia Standards, North Wales Dementia Strategy and any local strategies or new strategies launched. 

You will need to be able to use initiative to collect information from a range of sources and use this to support local citizens.  

Strong communication skills, confidence in the use of technology and confidence in working on a one to one, group basis or as part of a team are essential for this role.  

The team are adopting a hybrid working model of working in an office and working at home. You will need to be comfortable using technology to be able to work from any location and to maintain contact with colleagues. Currently, the team are based in Crown Buildings. 

An Enhanced DBS is required for this position.
