The post(s)
will be located within the Single Point of Access Team, Adult Social Care
Department, which comprises of First Contact Advisors, Social Care Assessors,
Social Workers, Occupational Therapist and access to Third Sector Agencies
and is based in the newly re-furbished
Crown Buildings, Chester Street, Wrexham.
Our Single
Point of Access provides the first point of contact most people have with Adult
Social Services and the dedicated team of multi-professionals provide a vital
service for the people of Wrexham. A call into the Single Point of Access for
residents or professionals means that they will receive high quality
information, advice and assistance. We are looking for highly motivated social
care assessors to play a key role within our friendly, supportive team.
You will work
in partnership with people to identify their needs, provide information, advice
and assistance and support them to plan creatively in order to increase
independence and reduce dependency on long term service provision. This will involve looking at solutions within
the persons own community, the third sector, specialist teams within the
department and our key partner agencies to ensure positive outcomes for the
person/ their family/carers.
You will be
supported by an experienced management team and a range of other professionals
and will have access to specialist training and development opportunities with
excellent career progression for the right candidates.
The Social
Services and Wellbeing (Wales) Act (2014), Mental Capacity Act 2005 and
Well-being of Future Generations (Wales) Act 2015 provide legislative
frameworks to ensure the team comply with statutory duties and that individuals
are supported to achieve positive outcomes.
The successful
candidates will have Level 3 or equivalent in Health and Social Care and
commitment to level 4 qualifications or equivalent within the required
timescales. The Department has recently introduced a ‘Grow Your Own’ programme
which gives employees the opportunity to work towards gaining a Social Work or
Occupational Therapist qualification.
