Permanent
Position - 37 hour post available
Part time hours

G10 Salary
£39,186 - £42,403 per annum
The Community Mental
Wellbeing Team is looking to recruit a dynamic, progressive, passionate,
experienced Social Worker who is committed to creating change and transforming
services to meet the needs of the citizens of Wrexham. If this sounds like you,

Our team
specialise in supporting citizens, experiencing mild to complex mental health
needs. We work in partnership with the Citizen, their families, and carers and
in collaboration with the Community Mental Health Team, Universal services and
fellow statutory agencies to get the best outcomes. We focus on the three tiers
of intervention, preventative service, and intermediate with short term
reablement services and complex care services with statutory interventions.
We work with
the Social Services and Wellbeing (Wales) Act (2014), the Mental Health Act
2007 and Mental Capacity Act 2005 and Safeguarding to ensure the team comply
with statutory duties and that citizens are supported to achieve positive
outcomes. We are looking for an Experienced Social Worker who has achieved
their Practice Educator and or Approved Mental Health Professional award. Experience
of working within a mental health service is desirable, but not essential, as
the successful candidate will be part of an experienced and supportive team.
We are looking
for an experienced social worker with a specific knowledge base and skill set;
- A strong person centred approach, with strengths based
assessments skills.
- Excellent advocacy skills to empower and promote
autonomy in problem solving and developing self-advocacy skills.
- Experienced in Hospital discharge processes.
- Expert knowledge and practice experience in Continuing
Health Care processes.
- Knowledge and practice experience in S.117 funding
processes and applications or a commitment to learn.
- Strong progression and reablement skills.
- Mental Capacity and Deprivation of Liberty practice experience.
- A qualified Practice Educator and or Approved Mental
Health Professional.

The successful
candidate will carry a manageable caseload and will undertake duty on a rota
basis, usually once a fortnight.
