G05 £24,294 - £25,119 per annum

 

An exciting opportunity has arisen to join the Community Outreach team as a Support worker, the new service will be community based following a full service review so this is a great time to join the team and be part of the transformational change.

You will work with adults of all ages who have a wide range of disabilities using a strength-based, person centred approach to understand and support people to achieve their personal outcomes that are meaningful to them both in the day centre and within the local community.

The successful candidate will need to be well organised, have exceptional communication skills and be a positive role model.

You will need to have excellent report and recording skills which are essential to ensure the delivery of person-centred care and outcome focused support plans to maximise independence and to give people choice and control over their lives.

You will work well as part of team and build upon effective relationships with the citizens who access the day service, colleagues, carers, other professionals and the wider community.

You will have a can-do attitude and demonstrate positivity and respect to the citizens who use the services.

An Enhanced DBS check & Social Care Wales registration is required for this position.

This Welsh essential post requires good conversational Welsh.  If you have not used your Welsh in a while don’t worry, further support can be given including access to accredited Welsh language courses. Applicants are encouraged to apply in Welsh however this is not essential as spoken Welsh is the key skill required in this particular role.
