An exciting opportunity has arisen in the Offender Assessment and Liaison Team, Adult Social Care. We are looking to appoint a highly motivated social worker to join our small and supportive team of Social Workers and Social Care Assessor.

 In response to referrals requesting Social Care input for residents of both HMP Berwyn and Plas-y-Wern Approved Premises, we undertake social work assessments and interventions to establish whether a resident has care and support needs. This role includes establishing appropriate support and care pathways for residents including the provision of information, advice and assistance. There will be a requirement to work collaboratively across agencies.

The team also work with individual’s that are subject to Multi Agency Public Protection Arrangements ( MAPPA ) that meet the criteria of specialist services, who may  require short/long  term social work assessment and intervention. This often involves working with a variety of citizens including those who may experience mental health issues, learning difficulties, self-neglect/hoarding or problems with substance use.

We are seeking a highly motivated Social Workers Grade 10. The right candidate for this position will have excellent communication skills, proven experience of working as part of a team, previous experience ofworking with adults and the ability to respond appropriately to  the needs of citizen’s/residents we support.

An Enhanced DBS check & Social Care Wales registration is required for this position.
