Care Broker (Adult Social Care)
G07 £30,559 - £32,115 per annum
1 x permanent 37 hours per week
An opportunity has arisen to join a busy and friendly team within the Social Services department on a permanent basis as a Care Broker. The team covers a range of support services – domiciliary care, residential care, supported living, day care and other services from third sector providers. You will be a point of contact for Providers, Social Work and Finance teams.
Your duties will include accurate and timely processing of care plan information and liaising with a range of providers and professionals to source the care needed for people in Wrexham. Strong communication and problem solving skills are essential as you will be dealing with a range of care providers and social work teams on a daily basis.
You will be need to be able to use initiative to collect information from a range of sources including online regulators’ websites (e.g. CIW, CQC) as well as contacting other local authorities. This role will involve sourcing care for adults and children. 
Strong numerical, Word and Excel skills are essential for this role as the team uses a range of spreadsheets and online databases. 
The team have adopted a hybrid working model of working in an office (Crown Buildings) and working at home. You will need to be comfortable using technology to be able to work from any location and to maintain contact with colleagues and must be able to attend the office on a weekly basis.
