Wrexham Disability Team are looking to recruit highly motivated Social Care Assessor. The successful candidate will be committed to contributing to the team’s multi-agency approach to providing a seamless health, social care and well-being service for individuals with disabilities of all ages, their families and carers.
The Social Services and Wellbeing (Wales) Act (2014), Children’s Act 1989 and Mental Capacity Act 2005 provide legislative frameworks to ensure the team comply with statutory duties and that individuals are supported to achieve positive outcomes at all stages of their lives.
You will work with individuals so that they are enabled to identify their own personal outcomes. Your work will focus on the individual and their support networks including families to promote independence, ensure the individual has a stronger voice in decision making and ensure they have more control in their lives.
Training and development opportunities will be provided to support the successful candidate in this exciting role. Successful candidates will be supported by an experienced team of practitioners and managers, and will received supervision and guidance.
The successful candidate will have Level 3 or equivalent in Health and Social Care and commitment to work towards a level 4 qualification or equivalent within agree timescales. The successful candidate will be based at the recently refurbished Crown Buildings within Wrexham City Centre. This role also offers the opportunity for hybrid working.
An Enhanced DBS check and Social Care Wales registration is required for this position.
